Consent to Telehealth Services
What you agree to when you receive care through HeydayMD by telehealth.

1. What telehealth is
Telehealth is the delivery of health care services using electronic communications — secure messaging, video or audio visits, and review of information and lab results you submit — when you and your clinician are not in the same place.
2. Who provides your care
HeydayMD is a technology platform, not a healthcare provider. Your evaluation and any treatment are provided by independent clinicians (physicians, nurse practitioners or physician assistants) licensed in the state where you are located at the time of your visit. Nurse practitioners and physician assistants work with a collaborating physician wherever state law requires. See Our Providers.
3. Benefits and limitations
Benefits include access to care without travel, at-home lab collection and ongoing follow-up by message.
Limitations and risks include:
- Your clinician cannot perform a hands-on physical examination, and may decide your condition is not appropriate for telehealth and refer you for in-person care.
- Decisions rely on the accuracy and completeness of the information you provide. Incomplete or inaccurate information can lead to an incorrect assessment.
- Technical problems (poor connection, device failure) can delay or interrupt care.
- Although we use safeguards, no electronic system is completely secure, and there is a small risk that information could be intercepted or accessed without authorization.
4. Privacy and security
Your health information is protected under federal and state privacy laws, including HIPAA. How your information may be used and disclosed, and your rights over it, are described in our Notice of Privacy Practices and Privacy Policy. Your visits will not be recorded without your permission.
5. Prescriptions
Completing an intake does not guarantee a prescription. A clinician prescribes only when, in their independent judgment, treatment is clinically appropriate and permitted by law. Controlled substances, including testosterone, are prescribed only in compliance with federal and state law, which may require additional evaluation.
6. Emergencies
Telehealth is not for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.
7. Your rights
- You may decline telehealth and seek in-person care instead, at any time, without affecting your right to future care.
- You may withdraw this consent at any time by contacting us. Withdrawal does not affect care already provided.
- You may request copies of your medical records as described in our Notice of Privacy Practices.
- You may ask questions about your care at any time through your care portal or by contacting us.
8. Your consent
You give this consent when you complete a purchase at checkout, which links to this page. By doing so, you confirm that you have read and understood this consent and agree that: you are at least 18 years old; you are located in the state you identify at the time of each visit; the information you provide is accurate and complete; you understand the benefits, limitations and risks of telehealth described above; and you consent to receive care via telehealth from clinicians made available through HeydayMD. Your consent remains in effect until you withdraw it.
