So what does true hormone optimization at forty-five actually look like?
It is not about finding the perfect supplement stack.
It is about understanding your specific hormonal status... identifying which systems have declined below functional thresholds... and applying targeted interventions backed by real clinical evidence.
Think about what is actually happening in your body right now.
At forty-five, multiple hormonal systems are in decline at the same time.
Testosterone falls by one to two percent every single year starting at age thirty.
Growth hormone and I-G-F one decline by roughly fourteen percent per decade after thirty.
D-H-E-A peaks in your twenties and drops steadily after that.

So, what actually works?
Evidence-based hormone optimization at forty-five follows four tiers.
First... you need the lifestyle foundations.
This means resistance training...
Optimizing your sleep...
Managing your stress...
And reducing body fat.
These are required whether you start medical treatment or not.
Second... you need a medical evaluation with comprehensive lab work to establish your baseline and find the deficiencies.
Third... you move to targeted treatment.
This might mean testosterone therapy for a confirmed deficiency...
Thyroid medication for hypothyroidism...
Or peptide therapies like sermorelin for growth hormone optimization.
Finally... the fourth tier is ongoing monitoring.
You need labs every three to six months to make sure the treatment is safe and effective.
Optimization is not a one-time event.
