Switching TRT clinics—what records and details should I bring?

Asked in r/Testosterone · Answered by the HeydayMD team ·

I’m considering moving my TRT care to a different telehealth clinic and want to avoid gaps or confusion about my current protocol. What information should I gather, and how does the new prescriber typically evaluate a transfer?

The short answer

Bring recent lab reports with dates, your current prescription and dosing schedule, injection or application details, other medications and supplements, and notes on symptoms, side effects, and relevant medical history. If available, include records showing how your dose has changed and why, along with the prior clinic’s contact information; don’t stop, change, or combine medications without guidance. The new prescriber will review your records, discuss your goals and safety concerns, and decide whether the information is sufficient or whether an evaluation or additional monitoring is appropriate. Requirements and prescribing decisions vary, so ask the new prescriber what documents they need and how to manage your current supply during the transition.
