Is free testosterone more important than total testosterone for TRT?

I recently got my labs back and my total testosterone seems okay, but my free testosterone is flagged as low. Which number should I be paying more attention to when deciding on TRT?

The short answer

While total testosterone is the most common metric, free testosterone is often more important because it represents the 'bioavailable' hormone actually able to exert cellular effects. If your total testosterone is normal but your free testosterone is low, it is often due to high levels of Sex Hormone-Binding Globulin (SHBG) or albumin binding up the hormone.
