Can I maintain my fertility while on TRT if I plan to have kids soon?

Asked in r/FYRbody · Answered by the HeydayMD team ·

I’m considering starting TRT for my low testosterone symptoms, but my partner and I want to have children in the next year or two. Is it possible to stay fertile while on treatment, or do I have to wait until after we conceive?

The short answer

Yes, it is possible to maintain or restore fertility while on TRT by incorporating Human Chorionic Gonadotropin (hCG) into your protocol. hCG mimics the hormone LH, which signals your testes to continue producing intratesticular testosterone and sperm even when external testosterone is present. At HeydayMD, our clinicians often prescribe a low-dose hCG protocol (typically 250–500 IU two to three times per week) alongside TRT to prevent testicular shutdown and keep your fertility options open. For those actively trying to conceive, we can adjust these dosages or discuss adding FSH-stimulating medications to optimize your results.

Every body is different. A licensed provider can look at your own labs and symptoms.
