Welcome to HeydayMD Health Talk. I'm your host, and today we're talking about why your free testosterone matters way more than your total number and the role of a protein called sex hormone binding globulin.
So... what is sex hormone binding globulin?
Most people just call it S-H-B-G.
It is a transport protein made by your liver that grabs onto your testosterone and locks it away from your tissues.
Think of S-H-B-G as a safety deposit box for your hormones.
The testosterone inside that box is still counted in your total number... but your body cannot actually use it.
Only the testosterone that is free or loosely bound to other proteins is available for your body to use.
This is why S-H-B-G is one of the most important and most overlooked markers in men's health.
So what does that actually mean in the real world?
Well... imagine two guys.
The first guy has a total testosterone of six hundred and an S-H-B-G of twenty.
The second guy also has a total testosterone of six hundred... but his S-H-B-G is sixty.
Even though they have the exact same total number... the first guy has far more usable testosterone.
It is a completely different clinical picture.
So why does S-H-B-G get high in the first place?
It can rise as we get older...
Or because of an overactive thyroid...
Liver disease...
Certain medications like anticonvulsants...
Low body weight...
Or even excessive endurance exercise.
High S-H-B-G is one of the most common reasons men feel the symptoms of low testosterone even when their total levels look normal.
It creates a functional deficiency.
The number on your lab report looks fine... but your body is operating with a deficit.
Now... what about low S-H-B-G?
This usually happens because of obesity...
Insulin resistance...
An underactive thyroid...
Or the use of certain medications like corticosteroids.
When S-H-B-G is very low... it can actually inflate your free testosterone calculations.
But it also causes your body to clear testosterone from the blood very quickly.
This leads to big fluctuations and "roller coaster" feelings between doses if you are on a protocol.
So how does this impact treatment?
Your S-H-B-G level directly changes the strategy for hormone therapy.
Men with high S-H-B-G often need higher doses or more frequent injections to keep enough free testosterone available.
On the other hand... men with low S-H-B-G might do better on lower... more frequent doses.
This helps them avoid high peaks that can convert into too much estrogen.
You might be wondering... can you treat S-H-B-G directly?
Not really. It is not like testosterone where you just add more.
But understanding your level helps optimize your plan.
Sometimes... high S-H-B-G can be lowered slightly with boron supplements... maybe six to ten milligrams daily.
This has been shown to reduce S-H-B-G by ten to twenty percent.
Addressing things like thyroid issues or liver health helps too.
Ultimately... the best strategy is adjusting your hormone therapy protocol to account for your S-H-B-G level... rather than trying to force the protein into a specific range.
